We offer support coordination across Western Australia. Our qualified, experienced Support Coordinators are familiar with local services and can give you the information you need to make the best choice for your plan.

We provide written reports, as required, on outcomes, successes and barriers, to support your participant funding reviews.

We work to strengthen and enhance your abilities so you can coordinate supports and participate in your community with greater independence over the longer term.
